When running a migration to Microsoft 365 while using Insane mode, you get one of these error messages on one or several of your items:

Details
You can get these errors when you migrate content to Microsoft 365 in insane mode.
When you migrate in Insane mode to Microsoft 365, your items are sent to Azure storage, and from there, they are imported to their destination in SharePoint online by Microsoft.
When an item gets a fatal error during the import process, Microsoft stops importing the whole package that the item was a part of. The migration tool reports the items in that package with errors in your migration report.
Often, you will get fatal errors when a package gets 100 errors during import. When this happens, Microsoft stops importing the whole package, including items that did not get an error.
A corrupt file is also often the cause of a fatal error from Azure.
For example, you might copy many items with unique permissions to a list and breach that list's 50,000 unique permissions limit. When that happens, each subsequent item will get an error, and when the import reaches 100 errors, you will get the fatal error messages on all the remaining items.

Solution
If you have fatal errors, you can try one of the solutions or workarounds below:
- Investigate and solve the fatal errors. While the app cannot provide more information about the cause of the fatal error, try downloading, opening, or moving the affected files manually. You might find they are corrupted or unreadable for various reasons.
- Look at your migration report for errors that might cause the fatal error.
- Skip the items encountering a fatal error and copy the other items only. You will have to use Copy content only to select the items that did not get the fatal error.
- Try migrating an affected list or library in Normal mode. While we do not recommend complete migrations in Normal mode, it is an excellent way to discover the underlying errors since this mode does not rely on Azure to send your data to the destination. Look at the migration report to solve these errors.
- If you are breaching the total amount of unique permissions, you can try restoring the inheritance of your items, or you can uncheck Permissions from the configuration section of your copy options. If you are required to preserve these permissions, you can split your content into multiple lists or libraries to avoid breaching the 50,000 unique permissions threshold.
